Using PJPP Forms

Always use original printed forms, or contact Ontario Pension Board (OPB), our Plan Manager, and they will send you the original form(s) that you need. This helps OPB to scan and record your information correctly and keep your records up to date. Please do NOT use photocopied forms.

Please PRINT if you complete the form by hand.

Completing PJPP Forms

Please follow these steps to complete and submit OPB forms:

  1. Open and complete the form online by typing your information into each field.
  2. Print two (2) copies of the form - one for mailing and one for your records. Note: You cannot save the form unless you have a licensed copy of Adobe Acrobat installed on your computer.
  3. Sign the printed copy of the form.
  4. Send the printed and signed form to OPB either by:
