Retirement forms
To help you officially start your retirement, we’ll send you a Retirement Information and Election package that outlines the key steps you need to take and forms you need to complete.
Below are some of the forms you may see mentioned in your package, though not all will apply to you:
- Personal Information Change Form - OPB1004 (PDF) and void cheque (for direct deposit of payment)
- Post-Retirement Insured Benefits Application - OPB1122 (PDF)
- TD1 Forms - Federal and Provincial
- Declaration of Spousal Status Form - OPB3007 (PDF)
- Proof of Relationship documents, such as marriage certificate, proof of common-law relationship - see Affidavit of Spousal Relationship - OPB3010 (PDF)
- Proof of age for member, eligible spouse, and eligible child
- If applicable:
- Proof of age for eligible spouse and children
- Family Law documents that specify how the pension was addressed on a marriage breakdown. Example: Separation Agreement, Divorce Documents (e.g., Divorce Decree or Divorce Judgement) or Court Order, if applicable
- FSCO Form 3 - Waiver of Joint and Survivor Pension(opens in a new tab)
- OPB1046 – Waiver of Basic Life Insurance Coverage Form (PDF)
- LTIP award letter
- Certificate of Indian Status (provided by the member, but issued by Indigenous and Norther Affairs Canada)
Your employer must also complete and forward these forms to us before you retire:
- A termination transaction through the OPS Employer Portal
- For a person with status under the federal Indian Act:
- Canada Revenue Agency TD1-IN form(opens in a new tab)(Determination of Exemption of an Indian's Employment Income)
- Letter confirming that the tax-exempt employment reported on the TD1-IN form corresponds to membership in the PSPP. Here is a sample letter (PDF)
